Audition Application Form



Completely fill out this Audition Application Form if you wish to schedule an audition for admission to the School of Music for the Fall 2008, Spring 2009 or Summer 2009 semesters. It is highly recomended that you submit your application at least three weeks prior to your requested audition date. Audition days often fill up and audition times are filled as applications are received. Auditions take place on the Tampa campus of the University of South Florida. DO NOT USE THIS FORM if you wish to simply perform in an ensemble. Separate auditions, which take place at the beginning of the fall and spring semesters, are required for placement in our music ensembles. If you wish to audition on more than one instrument you must submit a SEPARATE application for EACH instrument.
